Health Acknowledgment
I understand that massage therapy is intended for relaxation and general wellness purposes only. It is not a substitute for medical examination, diagnosis, or treatment by a qualified healthcare provider.
Medical Conditions Disclosure
I agree to inform my therapist of any medical conditions, including but not limited to:
- Cardiovascular conditions (heart disease, high/low blood pressure)
- Pregnancy or possibility of pregnancy
- Recent surgeries or injuries
- Skin conditions or open wounds
- Diabetes
- Cancer or history of cancer
- Blood clots or thrombosis
- Infectious diseases
- Allergies (especially to oils or lotions)
- Medications that affect blood clotting or sensation
- Any other condition that may be affected by massage
Contraindications
I understand that massage therapy may be contraindicated (not recommended) for certain conditions. I will consult my physician before receiving massage if I have any concerns about my health status.
Communication During Session
I agree to communicate with my therapist during the session regarding:
- Pressure preferences (too light, too firm)
- Any discomfort or pain
- Areas I prefer not to be massaged
- Temperature or positioning concerns
Assumption of Risk
I understand that massage therapy, like any therapeutic treatment, carries some inherent risks. These may include but are not limited to:
- Temporary soreness or discomfort
- Bruising (rare)
- Allergic reactions to oils or lotions
- Aggravation of existing conditions if not properly disclosed
Release of Liability
I hereby release Manila Wellness, its therapists, employees, and affiliates from any and all liability arising from massage therapy services, except in cases of gross negligence or willful misconduct. This release extends to any injuries or damages that may result from the massage session.
Professional Boundaries
I understand that Manila Wellness therapists provide therapeutic massage services only. The session will be conducted in a professional manner with proper draping at all times. Any inappropriate requests or behavior will result in immediate termination of the session.
Consent
By booking a session with Manila Wellness, I confirm that:
- I have read and understood this medical waiver
- I have disclosed all relevant medical conditions
- I am at least 18 years of age
- I voluntarily consent to receive massage therapy
- I agree to the release of liability stated above
